Connect Google Drive to start taking intakes
Intake forms hold Social Security numbers, dates of birth, Medi-Cal IDs, medical history and criminal background. None of that is ever stored on this dashboard — every answer is saved straight into your own Google Drive, in a folder called “Bethel Intake Forms”, and nowhere else.
That is also why the form stays locked until you connect: without Drive there is nowhere safe to put what you type.
For safety, a Google Workspace account is best.
Workspace can be covered by Google’s HIPAA business associate agreement,
which is what makes it appropriate for client health information. You can
connect any Gmail account you like, but a personal one is not covered by
that agreement — that choice is yours to make.
This dashboard asks only for permission to files it creates itself. It
cannot see, open or touch anything else in your Drive.
